Some people can recover without surgery and only need a splint, cast or sling. You’ll need physical therapy to regain your ability to move your arm. The surgical neck of the humerus is a bony constriction at the proximal end of shaft of humerus. It is situated distal to the greater tubercle and lesser tubercle, and proximal to the deltoid tuberosity. The surgical neck is much more frequently fractured than the anatomical neck of the humerus.

Proximal humerus fracture (PHF) includes all fractures to the anatomical neck, surgical neck, greater tuberosity (GT), or lesser tuberosity, either isolated or in combination.

Jul 28, 2016 · Two-part fractures most commonly involve fracture of the surgical neck with angulation or fracture of the greater tuberosity with displacement. Three-part fractures have a displaced and angulated humeral shaft in addition to one of the tuberosities displaced.

Proximal humeral fractures are proximal to the surgical neck (see figure Key anatomic landmarks in the proximal humerus). Most are minimally displaced and angulated. Diagnosis is by plain x-ray or sometimes CT. Most of these fractures can be treated with a sling, a swathe, and early mobilization. (See also Overview of Fractures.)

Fractures of the anatomic neck are very rare. In young patients, these fractures should be treated with open reduction and internal fixation. In elderly patients, prosthetic replacement would be preferred because of the risk of osteonecrosis.
